A new toolkit based on our Popup Survey Technique. Quickly create surveys that automatically pop-up as a page loads and go away when filled out. The Survey Builder generates both the client script for displaying the survey and the server script for processing the results.

Okay folks the reason you are getting the response error is that you need to put the following before the header in the submit.asp (or whatever you named it) page.

<%Response.Buffer=True%>

Very cool, excellent functionality. The only problem I have so far that it doesn't enter data into the results.txt

It looks great but it would be nice if you told us what to do with the ASP file. I believe you need to refer to the exact path in the javascript. I also wasn't sure if you could put an asp anywhere in the server. You seem to be able to...
Now I get this error
